Data sheet duplicates

Person applying for a duplicate of test report and decision issued for LPG container built in automobile is obliged to: 
  1. Give full company name (or name and surname in case of natural person), address with postal code, NIP (income tax identification number) or PESEL (Polish personal identity number), telephone number,
  2. Give identification data of the container - manufacturer, factory number, year of manufacture, serial no., capacity, homologation no. (given on the container),
  3. Pay the sum of 49.95 PLN to Technical Supervision of Transport, Bank Handlowy in Warsaw, account no. 19 1030 1061 0000 0000 5381 2001 with a note: "the payment for issuing of LPG container data sheet duplicate. Container no. ..."
  4. Attach a copy of the proof of payment to the application.
Application for issuing a duplicate of test report and decision issued for LPG container built in automobile should be delivered to:
